ONSITE ROLE IN NEWARK DE

• Community based program for SPMI adults, case management needs, collaboration with providers , assist with housing.
• The position performs a fundamental role in the discipline management, recovery mission and compliance with inspection and audit criteria.
• The Social Worker will interview clients and family members to obtain detailed social history.
• They will connect and collaborate with providers to locate housing and other vital community supports.
• Other responsibilities include admissions tasks such as social assessments, interventions, developing and implementing plan goals/interventions.
• Discharge planning coordination to support successful re-integration into the community.
• The candidate must have experience with documentation, report writing, participating as a professional member of an interdisciplinary team.
• Candidate must have a Bachelor’s degree or higher.

Valid driver’s license required
