Job Description

This Registered Nurse position requires providing skilled nursing care, case management, and hospice care to patients facing age-related frailty and serious illness. The role involves patient assessments, medication administration, treatment planning, and collaboration with an interdisciplinary team. A minimum of two years of experience is required.

Location: Tarzana, California, United States,

Responsibilities:

- Patient assessments and monitoring.
- Medication administration and documentation.
- Treatment planning and implementation.
- Collaboration with physicians, other nurses, and healthcare professionals.
- Case management and coordination of care.
- Providing skilled nursing care in various settings (potentially including hospital, home health, or hospice).
- Maintaining accurate patient records.

Required Skills & Certifications:

- Clinical assessment skills.

- Excellent communication and interpersonal skills.
- Active Registered Nurse license in the state of employment.
- Bachelor's degree in Nursing or related field.
- Minimum two years of experience in hospital, home health, or hospice care.
- Driver's license, car insurance, and personal automobile for field work.

Preferred Skills & Certifications:

- Experience in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room settings.

Special Considerations:

- On-call responsibilities are expected 1-2 days per week and rotating weekends.
- First-time nurses are accepted.
- Must be able to work in a fast-paced environment.

Scheduling:

- Schedule will include rotating weekends and on-call shifts (1-2 days per week). Specific shift assignments will be determined by the employer.
