Erie Family Health Centers, a top workplace in Chicago and suburbs, is looking for a valuable addition to our Care Management Team! The Licensed Care Manager ensures the delivery of comprehensive, high-quality, patient centered care management services for Erie Family Health Centers (Erie’s) high risk patients. Key responsibilities include conducting face-to-face and telephonic patient interactions, completing risk assessments, developing care plans to directly impact patient outcomes, helping patients learn to manage their conditions and providing needed psychosocial resources for a panel of 75 patients. The new team member will also work directly with providers, and other clinical staff to promote timely health care delivery, preventive care, self-management support, medication management, and discharge planning, and to address clinical or psychosocial barriers to achieving the goals of the plan of care.
