About the Role

This position includes a sign-on bonus, payable according to company policy and contingent upon successful completion of required employment conditions.

Responsibilities

• Conduct and document patient health histories
• Assist providers in delivering high-quality, patient-centered care
• Manage patient flow, including triage and follow-up care
• Coordinate referrals to specialists and community services
• Provide health education to patients and caregivers
• Collect lab specimens (e.g., blood, urine, sputum)
• Support infection control, safety standards, and supply use
• Occasionally assist with front office duties (scheduling, discharge, etc.)
• Float between Community Health Net locations as needed

Qualifications

• Valid Pennsylvania LPN license & current CPR certification
• High school diploma or GED

Required Skills

• Strong assessment, organizational, and communication skills
• Knowledge of medical terminology and basic insurance types
• Ability to use computers and electronic health record systems
