Job Location : Johnstown, NY.
Job Schedule: TBD Start late May/June 2025 with ongoing opportunity.
Job Type: Per diem, 1099 contract.
Pay Rate: $80/hr.

Full Description

VitalCheck Wellness is seeking an experienced Physical Therapist to evaluate employees of a private business in Johnstown, NY. This is a great opportunity for physical therapists who desire to help improve the physical function, work performance, and daily lives of employees of a manufacturing company with multiple sites.

• Conduct initial in-person assessments, including demonstration and instruction of exercises tailored to individual needs.
• Perform ergonomic assessments.
• Provide follow-up care both onsite and virtually.
• Develop personalized treatment and exercise plans based on assessment findings.
• Identify when further medical or rehabilitative care is needed and refer appropriately.
• Maintain timely and accurate client documentation.
• Deliver home exercise programs and provide guidance through telehealth as appropriate.

Requirements:

• Masters Degree from a Physical Therapy program accredited by the Commission on Accreditation in Education of the American Physical Therapy Association is required.
• Clean Active Physical Therapist License in state of NY.
• At least 3 years of experience as a Physical Therapist.
• Proficient with computers, Microsoft Office, EMR systems.
• Excellent communication skills.
• Ability to work independently with little supervision.
• Personal laptop or tablet required.
• Must be able to reliably commute to office in Johnstown, NY .
