The Quality Assurance Manager is responsible for planning and overseeing a comprehensive quality assurance program to ensure compliance with federal and state mandated quality standards for Samaritan Home Health. The incumbent performs a variety of compliance functions to assure the agency is prepared for Medicare and Medicaid audits and the New York State Department of Health (NYSDOH) survey at all times. These functions include staff development, in-service training, quality assurance, patient review, credentialing, and competency testing, supervision and evaluation of field staff. The incumbent will conduct and coordinate policy review, development, and implementation subject to standards of best practices. The work is performed under the direct supervision of the Director of Patient Services at Samaritan Home Health. The incumbent is expected to work as a team member in promoting excellence in nursing standards. In the absence of the DPS the incumbent will act as stand in DPS. Provide patient care and case management as assigned. On call as required
Duties and Responsibilities /Performance Improvement: Assists case managers in the interpretation of the policies, procedures, philosophy and objectives of nursing services. Assists case managers in assessing the nursing needs of the patients and delivery of care. Conducts joint supervisory visits with staff as directed by the DPS. Participates with inservice/education programs. Participates in the development of nursing personnel. Assists in the maintenance of patient care records through chart audits and reports findings to DPS and Professional Advisory Committee. Provides clinical support and supervision for all patients and participants in the CHHA. Responsible for completing chart audits on an on-going bases as assigned by Director of Patient Services. Responsible for reviewing monthly home health quality indicator reports to determine chart audits and agency trends. Participates in agency’s Professional Advisory Committee and Performance Improvement meetings and reviews audit reports with all in attendance. Provides supervision of RN Case Managers, Physical Therapist, PTAs, LCSW, Occupational Therapist, COTAs, Speech Therapist, Nutritionist, HHAs, LPNs by providing Generic supervision of services and completing employee evaluations and progressive discipline per policy and regulations in collaboration with DPS. Will perform other duties as assigned by the Director of Patient Services.
Job Requirements Minimum Education Requirement: Graduate from an accredited Nursing Program.
Minimum Experience Requirement: Previous experience within a Home Health Agency required. Knowledge of Nursing and Medical practices and procedures, as well as laws, regulations and guidelines that pertain to home health care. Familiarity with Medicare/Medicaid and payor regulations. Ability to plan, organize, develop, implement and interpret the programs, goals, objectives, policies and procedures that are necessary for providing quality care and maintaining a sound operation. Ability to demonstrate knowledge of computer systems, system applications and other office equipment.
Minimum License Requirement: Current NYS RN license. Valid driver’s license.
